        I hope this letter finds you happy and healthy. I am writing to inform you of an upcoming change. After careful consideration, I have decided to transition my career outside of Southwest Surgical Associates, L.L.P.

Thursday, March 31, 2022, will be my last day seeing patients at Southwest Surgical Associates, L.L.P.

It is very important to me that you maintain continuous health care. You have the right to make your own healthcare decisions, including the right to decide where you receive medical care. To maintain continuous health care, you will need access to your medical records.

You may obtain copies of your medical records, or request to have your medical records transferred to a physician of your choice, by contacting Southwest Surgical Associates, L.L.P at (713) 772-1200.
